1. Mark the cervico-occlusal dimension of the crown and the root on the wax block. The portion of the wax block beyond the root is called the base. Label the surfaces at the bottom of the block as B, L, M, and D indicating buccal, lingual, mesial, and distal.

2. On the occlusal aspect, mark the two line angles as shown in the picture( obtuse monolingual line angle and acute distobuccal line angle). The marking should eventually make the crown wider lingually than buccally and mesially wider than distally. Remove wax outside the markings.

3. Mark the mesial and distal outlines and the slopes of the buccal and lingual cusps carve wax outside the markings. Create the cusp tips.

4. Occlusal surface: Carve the triangular ridge of the distolingual cusp and the mesiobuccal cusp. Carve the distobuccal cusp and the monolingual cusp with the intervening oblique ridge and subsequently the transverse ridge.

5. Mark the outline of the root on the wax block. There are three roots which are the mesiobuccal, distobuccal, and lingual root. Carve the wax outside the outline which has been marked (shaded portion).

6. Check for the dimension and complete carving the anatomy of the tooth in detail. Carve the cervical line and polish the tooth.

The steps in carving the permanent maxillary second molar are the same as the permanent maxillary molar. The exceptions are mentioned below

  1. Outline
  2. Dimensions of the crown and the root.
